Facility Setup Forms
If you have reserved the Spiritual Center, Life Center or Parish Hall for a minstry meeting/gathering, please PRINT, COMPLETE and SUBMIT the appropriate form 2 weeks BEFORE the date of your event to the Church Office. You may scan and email to CHURCH@SAINTV.ORG, mail to "1502 E Wallen Rd, FW, 46825" or drop off the completed form at the Church Office (there is a mail slot in the office door if the office is closed).
